Participant information and consent 

 

You are invited to participate in a research project which seeks to identify the impact of COVID 19 on safety performance in the mining industry in Western Australia. You have been selected to participate in the research because you are a safety professional working in the mining industry in Western Australia.  

 

Your involvement in this research is voluntary and you can cease to participate at any time.  The information you provide is anonymous.   

 

By answering question one of the survey, you agree to participate and acknowledge that you:

·       have been provided with a copy of the Participant information & consent letter, explaining the research study 

·       have read and understood the information provided 

·       have been given the opportunity to ask questions and have had questions answered to your satisfaction

·       can contact the research team if you have any additional questions

·       understand that participation in the research project will involve completing a survey

·       understand that the responses are anonymous

·       understand that you are free to withdraw from further participation at any time, without explanation or penalty

·       freely agree to participate in the project
