Ageing Well in Wanneroo

 

Thank you for your interest in this research project which will help the City of Wanneroo deliver relevant services for people aged 60 years and older. This study has been approved by the Edith Cowan University Human Research Ethics Committee

 

Please read the attached Ageing Well in Wanneroo Information Letter, and then to confirm your consent in this research click 'Yes', and then to proceed click on the arrow below. In doing this, you will confirm the following statements, and be directed to the online questionnaire:
 

 

• I have been provided with an overview explaining the research project.  

 

• I have been given the opportunity to ask questions and have had any questions answered to my satisfaction.  

 

• I am aware that if I have additional questions I can contact the Chief Investigator or a representative from the City of Wanneroo.  

 

• I understand that participation in the Ageing Well in Wanneroo Project will involve the completion of an anonymous, confidential questionnaire.  

 

• I understand the information provided will be kept confidential; that the identity of participants will not be disclosed without consent; and that all information will be securely stored for at least five years before being destroyed.  

 

• I understand the information provided will be used for the purposes of this research project, and I understand how the information is to be used.  

 

• I understand that findings from this research may be used in future projects aimed at promoting older people’s involvement in community-based programs.
